Field Usage Request - Duval County Only

Sporting Jax Soccer Academy has a lease with the City of Jacksonville fields which gives the exclusive use of the fields to Sporting Jax. In turn, Sporting Jax provides the year-round care and maintenance for each of the fields.

As per our agreement, any group who would like to sub-lease the fields has to be a non-profit entity. If your organization meets this first criteria, then you must complete the following:

1. Provide proof you are a registered non-profit entity in the State of Florida.
2. Complete a field request form and get written notification of permission from Sporting Jax Soccer Academy.
3. Provide Sporting Jax and the City of Jacksonville with a copy of a liability insurance policy meeting all the City requirements, and showing both the City and Sporting Jax as certificate holders.
4. Pay a field usage fee for the benefit of the care and maintenance of the fields.
